Referral Process

Referrals may be submitted by fax with:

• Patient demographics
• Insurance information
• Most recent labs
• Dialysis center information (if applicable)
• Relevant imaging or prior operative reports

For urgent access issues (thrombosis, non-functioning access, catheter malfunction), please contact our office directly for expedited evaluation.

Communication

Operative reports and procedure notes are sent promptly following intervention. We remain available for ongoing collaborative management and access planning.
